The crew breaks out the old concrete, hauls it away, digs out any bad fill and compacts a fresh base. Then they set forms, add reinforcement where the slab needs it and pour new concrete pitched to drain away from your foundation wall and toward a proper outlet.

Row house blocks share walls and back fences, so noise and dust reach next door quickly. A short heads up to neighbors usually prevents complaints and keeps the day smooth.
Watch for plants you want to keep. Garden beds along the edge of an old slab get trampled during demolition unless they are marked. Point them out and the crew will work around them.
